HIV 

Alzheimer’s disease

Amyotrophic lateral sclerosis

Cancer

Cachexia

Persistent nausea 

 cyclical vomiting syndrome,

 cannabinoid hyperemesis syndrome

Crohn’s disease

 ulcerative colitis

Epilepsy or debilitating seizures

Multiple sclerosis

muscle spasms

Post-traumatic stress disorder (PTSD) 

Autism

Severe or Chronic pain Lasting longer than 2 weeks

A terminal illness when the patient’s life expectancy is less than six months

A condition resulting in the individual receiving hospice care

A condition that the compassionate use board approves (once established) on a case-by-case basis

Patients with medical conditions not listed in the law may petition the Compassionate Use Board for a medical card.

When setting appointment please bring your Utah Driver's License, and proof of your qualifying medical condition. This can be Medical Records, MRI Report or a letter from your doctor.  Please check with us if you have other medical documents not listed

In-Clinic appointment $200 Recertifications can be done by telemedicine only if you were certified by us before, otherwise you must make an appointment to come in-person. Recertifications are $125 if you are our patient, New Patient Recertifications are $175

Patient Card (six-month renewal): $15
